Sheridan Hollow Housing

In 2009, Interfaith constructed 8 beautiful one bedroom apartments and 2 two bedroom family apartments to provide permanent housing for chronically homeless individuals and families with a disabled head of household. Tenants residing in these apartments receive ongoing case management from the clinical housing case manager who is conveniently located on site. Tenants' rent is subsidized though the Shelter Plus Care Program. Participation in both the case management service and Shelter Plus Care Program are required for the duration of tenancy.

Eligibility Criteria: single units must be chronically homeless (HUD defined) with a disability documented by a physician, family units must be homeless (HUD defined) with a disability documented by a physician.

          Homeless Prevention and Rapid Rehousing Program

This Program is a collaborative effort between the City of Albany and multiple local agencies.  Interfaith's role is to receive referrals of individuals currently in housing who are facing eviction, yet have been deemed eligible by United tenants to receive a monthly rental subsidy for a maximum of 12 months.

Interfaith's case manager will provide ongoing housings case management for participants, monitor the monthly rental subsidy, and complete recertification with all participants every three months.  In addition to general case management activities, the case manager will focus in on budgeting skills and ways to increase or maximize income, in an efforts to promote greater self-sufficiency.

Eligibility Criteria: all cases must be referred by UTA, LASNNY, or Catholic Charities after receiving emergency eviction prevention assistance.

Hope Through Housing

This HUD-funded program provides a portable rental subsidy for single homeless individuals with a disability, as well as ongoing case management for the duration of the subsidy. Rental subsidies are based on 30% of the income and can be used for a one bedroom apartment in Albany County.  The housing case manager will maintain regular contact with the individuals, provide life skills education, ect.

Eligibility Criteria: Must be homeless (HUD defined) with a disability documented by a physician.
