Job Opportunity: Full-time Case Manager

Interfaith Partnership for the Homeless (IPH) is a progressive agency that offers programs and services, including a 30-bed emergency shelter, Drop-In Center, housing programs and a summer youth program. IPH helps individuals and families by meeting their immediate needs, providing case management services to address individual factors of homelessness so they are able to find safe, affordable housing. IPH also offers ongoing support through long term case management services to assist individuals in maintaining their housing and building a stable lifestyle.

We are looking for a talented team player with general knowledge in systems, communicating with people, and a can-do attitude.  IPH is currently accepting resumes for a Case Manager with experience. This is a ‘Full-Time’ opening; weekdays, 11:00am-7:00pm. In this non-exempt role, you will have the opportunity to help support our guests and the Agency’s Mission.

Position Profile / Outline

• Meet with guests weekly to monitor and support their individual progress toward goal attainment and their adherence to shelter guidelines
• Accept referrals, conduct intakes in accordance with IPH policies and procedures
• Develop goal plans with guest participation
• Ensure appropriate referrals, advocacy, and monitoring of goal progress are reflected in documentation and milestone achievement.
• Maintain compliance with Department of Social Services documentation requirements for payment of individual shelter stays
• Make referrals for healthcare, drug/alcohol treatment, clothing, employment and educational services, and disability support services, as appropriate
• Provide more frequent contact and intensive case management for guests with more complex needs until appropriate support services can be accessed
• Provide crisis intervention when appropriate
• Ensure documentation of case management services provided
• Conduct weekly Life Skills Groups and coordinate on-site and off-site activities for guests.
• Meet weekly with supervisor for case review and supervision
• Other assigned activities that build the capacities of guests and staff to fulfill the mission of IPH.


1. Associate’s Degree in Human Services or related field required, Bachelor’s Degree preferred
2. Valid New York State Drivers License and reliable transportation required
3. Basic familiarity with computers: Windows, MS Office, email, and database use
4. Good working knowledge of local resources

  • Obtain successful pre-employment PPD test and maintain annually.
  • Perform & successfully pass Drug Screenings as required
  • Ability to honor and maintain strict confidentiality in all areas: written, spoken and observed communication
